American Heart Association Issues New Heart Disease Guidelines

Healthcare professionals should focus on women’s lifetime heart disease risk, not just short-term risk, according to updated American Heart Association guidelines.

The 2007 Guidelines for Preventing Cardiovascular Disease in Women – published in a recent special women’s health issue of Circulation: Journal of the American Heart Association – also include new directions for using various therapies, such as hormone therapy and vitamin and mineral supplements in heart disease and stroke prevention in women.

The updated guidelines emphasize the lifetime risk of women, not just the more short-term focus of the 2004 guidelines.

The guidelines take a long-term view of heart disease prevention because the lifetime risk of dying of cardiovascular disease (CVD) is nearly one in three for women. This underscores the importance of healthy lifestyles in women of all ages to reduce the long-term risk of heart and blood vessel diseases.

The guidelines include a new paradigm for risk assessment based on risk factors and family history, as well as the Framingham  risk score.  (First published in 1998, the Framingham risk score estimates the risk of developing coronary heart disease within 10 years.)

The new guidelines include expanded recommendations on lifestyle factors such as physical activity, nutrition and smoking cessation, as well as more in-depth recommendations on drug treatments for blood pressure and cholesterol control.

Some of the updated recommendations include:

  • Recommended lifestyle changes to help manage blood pressure include weight control, increased physical activity, alcohol moderation, sodium restriction, and an emphasis on eating fresh fruits, vegetables and low-fat dairy products.

  • Besides advising women to quit smoking, the 2007 guidelines recommend counseling, nicotine replacement or other forms of smoking cessation therapy.

  • Physical activity recommendations for women who need to lose weight or sustain weight loss have been added – minimum of 60–-90 minutes of moderate-intensity activity (e.g., brisk walking) on most, and preferably all, days of the week.

Small Business Health Plans in the 110th Congress

In the 109th Congress, the ACA successfully won the battle of Association Health Plans (AHPs) when S.1955, the Health Insurance Marketplace Modernization and Affordability Act (HIMMA), was withdrawn from the Senate floor and defeated along a party-line vote.  HIMMA attempted to expand health care access and reduce insurance costs through the creation of small business health plans.

Small business health plans, often called association health plans, or AHPs, offer insurance coverage purchased through guilds on behalf of the small businesses that make up their memberships.

The controversial small business health plan legislation spurred an unprecedented grassroots lobbying campaign by the American Chiropractic Association and hundreds of provider and consumer groups that believed the bill would gut state patient protection laws and leave millions of patients without crucial health care benefits.

The ACA expects to see similar legislation introduced again in the 110th Congress and we are ready to fight once more to protect the rights of chiropractic patients.

Chiropractic Usage

A study in the online journal www.ChiroandOsteo.com, provided a descriptive review of the scientific literature examining use rates of modalities and procedures used by CAM clinicians to manage chronic LBP and other conditions.

A literature search of PubMed and MANTIS was performed using the key terms Chiropractic; Low Back Pain; Utilization Rate; Use Rate; Complementary and Alternative Medicine; and Health Services in various combinations.

The study concluded that a review of studies looking at chiropractic utilization demonstrate that the rates vary, but generally fall into a range from around 6% to 12% of the population, most of whom seek chiropractic care for low back pain and not for organic disease or visceral dysfunction. CAM is itself used by people suffering from a variety of conditions, though it is often used not as a primary intervention, but rather as an additional form of care. CAM and chiropractic often offer lower costs for comparable results compared to conventional medicine.

Health Corner –In this edition of HealthBeat’s Health Corner, we present a discussion from the PBS show NewsHour with Jim Leher.  The report focuses on current research which suggests a person's positioning on the social hierarchy ladder is intimately related to his or her risk of health and disease.
